Definitions:
Provisional students are defined as:
     • Those who are listed under McKinney-Vento, not up to date with vaccines but are being assisted to update their immunizations will be documented as provisional. Please note that these students should be helped as soon as possible to obtain immunizations- due to circumstances, these students can be at higher risk for disease. There is funding provided to help with appointments and transportation through the McKinney-Vento liaison in your school.
     • The other provisional students would be those who presently have received every vaccine that they can receive and are waiting on spacing time frames to complete the series. These students also have a medical plan signed by a physician for the vaccines that spacing is needed to complete. They are medically up to date but not fully vaccinated. This does not include those with just an appointment and not waiting on spacing needs or those who have a signed note from a physician that they are up to date when they are not.

Medical exemption- A medical exemption is defined by a note from a medical provider, MD, DO, CNP, that states that receiving a vaccine or vaccines could be detrimental to the health of the child. This is not those who have a note signed by a medical provider stating that they are up to date when they are not.

* Children who are not up to date, either by doctor’s note stating that the student is up to date when they are not or by non-vaccination, and are attending school, will be listed as non-compliant and attending on the SILR.

NOTES:

1. If a student had three doses of Polio, the last dose being given after the fourth birthday and six months from the previous dose, please count it in the 'Polio 4 Doses or more column'. The student is considered up-to-date based on the catch-up schedule per the CDC guidelines.

2. The Meningococcal vaccine (MCV) is a 2-dose series 11-12 years (required for entry to 7th grade) and 16 years, (required for entry into 12th grade.) A dose received at 16 years or older shall count as the 12th grade dose.
