Please note the following:

  • A valid phone number and email address are needed for contact purposes.
  • Usernames and Passwords are case sensitive.
  • Passwords must be at least 8 characters long and no more than 24 characters.
  • Passwords must contain 3 out of the following 4: (1) CAPITAL LETTER, (2) lower case letter, (3) special character, (4) number.
  • Passwords cannot be the same as your username, first name, or last name.
  • Passwords cannot be the same as any of the previous passwords you used.
  • Passwords expire after 60 days and must be changed in order to gain access to this system.

Changes to your account (including adding more schools) may always be completed at a later time, by first logging in and then clicking the My Account link. Always verify the schools you are responsible for prior to survey data entry.
